Community Care Services

The Community Care Services Program (CCSP) provides services to frail, elderly individuals so they can remain in their own home or community as an alternative to nursing facility placement. The services available under this waiver are:

  • Adult day health
  • Alternative living services (personal care homes)
  • Homemaker
  • Personal support services
  • Respite care
  • Emergency response
  • Home delivered meals
  • Skilled nursing care
  • Home health aide services

Independent Care Waiver

A waiver from the Health Care Financing Administration authorizes Medicaid coverage of services to eligible severely physically disabled individuals who are medically stable, but are in a hospital,nursing home, own home, or are in risk of being placed in a facility.

The Independent Care Waiver is a community-based, long-term care program, which provides Medicaid funding for individuals who are functionally impaired as an alternative to institutionalization. The program is designed primarily for clients who have Traumatic Brain Injury (TBI) or Spinal Cord Injury. The program provides a variety of services which includes, but not limited to, 24 hour care givers in the clients own home, home modification to equip the home for client access, the purchase of medically approved equipment (Example, electric hospital bed, electric wheel chair, etc).

Alternative Living

With Alternative Living licensed personal care homes provide a viable option to the family who may need a nursing home alternative. This alternative offers 24-hour supervision, room and board, and companionship in a state approved private home. The resident receives personalized care in a home setting with fewer residents. Residents receive bi-monthly nursing visits from a designated provider agency.

Homemaker Service

This program provides assistance with light household chores, meals, shopping, and general related services for the resident who may be either temporarily or permanently disabled.

Respite Care

Respite care is in-home relief for family of consumers who cannot be home alone. Our trained and licensed staff allow the primary care giver time away from home to attend to personal business, the needs of the consumer away from the home, or temporary relief for the care giver.

Personal support refers to provisions of personal assistance such as assistance with or supervision of activities of daily living such as: bathing, dressing, toileting, transferring in / out of bed or chair, and eating. It also refers to medically related activities such as: reminding persons to take their medication, arranging and accompanying consumer to medical appointment, observing and reporting changes in condition to RN supervisor. Caregivers encourage proper nutrition, preparation of meals for consumer, and some grocery shopping.

Structured, comprehensive, health, social recreational experiences in a day care center. Services also include field trips and opportunities for partnering with varied community resources for appropriate activities. Supervision for the center and all services under the direction of RN/LPN.

S.O.U.R.C.E. (Service Options Using Resources in Community Environments)

The Source Programs blends primary medical care with preventive and supportive services. It is designed primarily for the older and disabled persons with chronic health conditions who are eligible for Medicaid and or SSI (Supplemental Security Income). Persons under the age of 65 may be eligible if they have significant disabilities. Services are provided through “enhanced case management” with the purpose of improving health outcomes, and delaying or preventing nursing home placement.
